Lower Blepharoplasty (Eyelid Surgery), Facial Fat Injections. Our 54-year-old female patient underwent a lower blepharoplasty procedure to eliminate her under eye wrinkles, remove some excess skin, and smooth over the lower eyelid skin for a younger look. Lower eyelid surgery can sometimes be performed without leaving an external scar. Fat is removed using an incision within the internal portion of the lower eyelid (transconjunctival blepharoplasty), to correct any bagginess. Skin tightening is then performed using a laser or chemical peel. The major advantage of making the lower eyelid incision on the inside of the lid is the avoidance of trauma to the muscle that supports the lower lid. This is important in preserving the natural shape of the eye after surgery to prevent a “surgical stare” appearance. The carbon dioxide, erbium laser or a TCA peel may also be used to resurface the lower eyelid, causing a mild contraction or shrinkage of the excess skin. After the blepharoplasty procedure, the patient underwent autologous fat transfer to her face using her own steralized fat. Dr. Tehrani injected fat to her malar region, nasolabial folds, marionette lines and lips. After the procedure, the patient felt like a new woman!
Dr. Kevin Tehrani, MD, FACS, is certified by the American Board of Plastic Surgery and is a fellow of the American College of Surgeons. He earned his medical degree at SUNY Downstate Medical Center, and became Chief Resident in General Surgery during his residency at New York’s Beth Israel Medical Center. Dr. Tehrani holds the academic position of Assistant Clinical Professor in the Department of Surgery at SUNY Downstate Medical Center, and proudly served as a Lieutenant Commander in the Naval Reserves Medical Corps.
