Breast augmentation has provided many women with increased self-confidence, better figure proportion, and improved body image. This surgical procedure enhances the size and shape of the breasts by inserting implants under or over the pectoral muscles.

Who is a Candidate for Breast Augmentation?
Candidates include women who feel their breasts are too small, women who have experienced a reduction in breast volume after pregnancy, or women with breasts that do not match in size or shape. Breast augmentation is a common choice after pregnancy and breastfeeding, when breasts often lose their original and youthful look and shape.
The surgery is performed on adults, usually no sooner than age 17 or 18. Rarely, the surgery may be performed at an earlier age if the patient is suffering emotionally from extreme asymmetry of her developing breasts.
The Breast Augmentation Procedure
Your surgery is usually performed under general anesthesia on an outpatient basis. Incisions can be placed under the natural crease of the breast or around the areolas that surround the nipples. Dr. Tehrani generally places the implants under the muscle, which he believes offers his patients a better result, including less risk of a scar capsule developing around the implant.
Another technique outlined below is the TUBA procedure. Dr. Tehrani will discuss the incision and placement options with you prior to your surgery and make recommendations that are specific to your case.
While saline implants are most commonly used, silicone gel implants are also FDA-approved and available. Dr. Tehrani will help you choose the right implants for you based on your anatomy and how you wish to look. Read more about implant choices below.
Breast Augmentation Before and After Photos
The TUBA Procedure (Trans-Umbilical Breast Augmentation)
The transumbilical technique, or TUBA, is unique in allowing Dr. Tehrani to insert the breast implants through a small incision in the navel rather than in the underside of the breast, under the arm, or around the nipple. Currently, only saline implants can be placed using the TUBA technique.
The implants are still deflated when inserted; they are moved into position under the muscle behind the breasts, and then filled with saline (salt water solution) to the predetermined volume. Dr. Tehrani is an established practitioner of this technique in Manhattan and Long Island.
Recovery from Breast Augmentation
After your surgery, you will experience some swelling, bruising, tightness and soreness. We will provide you with a surgical bra that you must wear for a few weeks, as well as pain medication to make you more comfortable as you heal. We will also provide you with full post-op instructions for washing your stitches and when to come back to our office for follow up appointments.
Most of our patients choose to take 1-2 weeks off from work after surgery. You can return to exercise within a month to 6 weeks.
Types of Breast Implants
Breast implants come in many shapes and sizes, with differing inner and outer materials. Together, you and Dr. Tehrani will select the option that is best suited for you and most likely to achieve the results you desire.
All breast implants have an outer pliable envelope (or shell). The contents of the envelope are used to achieve fullness and create the most natural breast-like feeling possible. The implants are filled with either saline or silicone gel. Saline is salt water, much like the fluid that makes up 70% of the human body. If the implant should leak and the saline is released, it can be safely absorbed by your body. Silicone gel-filled implants are also FDA-approved and very safe.
The implant shapes are generally round, oval, or contoured. The surface of the implants can be smooth to the touch or textured. Dr. Tehrani will also discuss these options with you.
With the FDA approval of new implants, there are more choices than ever. Aristocrat Plastic Surgery has undergone a rigorous certification process to gain access to the wide range of breast implant options in Allergan’s INAMED® Matrix of breast implants for augmentation and reconstruction surgeries. INAMED implants are the first anatomical-shaped cohesive silicone gel products on the market.
Allergan also has a newly FDA-approved implant called NATRELLE INSPIRA™ – a silicone implant with a higher fill ratio, nearly 300 choices of sizes and shapes, and an exceptionally durable shell to decrease the risk of leakage. There are two varieties of INSPIRA implants – one with a more natural feel and one that is ultra-cohesive to prevent rippling. We also offer Sientra’s silicone fifth-generation breast implants in both round and teardrop shapes, as well as IDEAL IMPLANT® Structured Breast Implants. IDEAL implants are filled with saline but designed to have a more natural feel like silicone. They include lowered edges to contour to the chest wall and several implant shells nested within one another. This internal structure reduces bouncing and movement of the saline filling. These additional options ensure that you will get the best possible result from your breast augmentation surgery.
